BUSINESS LICENSE APPLICATION CHECKLIST
Listed below are the documents required to be submitted with a medical cannabis business license application packet.
Please note that all documents on the checklist must be provided at time of submission and incomplete applications
will not be accepted by the Alcoholic Beverage and Cannabis Administration.
Applicants are strongly encouraged to review their applications thoroughly prior to submitting.
□ Medical Cannabis License Application
□ Trade Name Registration
Department of Licensing and Consumer Protection (DLCP) – Visit dlcp.dc.gov/node/1619191
□ Certificate of Good Standing for the Corporation
Department of Licensing and Consumer Protection (DLCP) - Visit corponline.dlcp.dc.gov
□ Certified surveyor’s report or detailed Geographic Information System (GIS) map detailing proximity of proposed
location to schools or recreation centers (Not required for Courier applicants)
□ Valid zoning determination letter or valid issued Zoning Certificate as determined by the D.C. Office of Zoning. (Not
required for Courier applicants)
□ Clean Hands Certification for the business and all owners
Certification is issued by the District’s Office of Tax and Revenue (OTR). - Visit mytax.dc.gov.
□ Medical Cannabis Acknowledgment and Attestation Form
□ Medical Cannabis Social Equity Declaration Form
□ Social Equity Applicant Attestation Statement (if applying as a Social Equity Applicant)
□ Medical Cannabis Certified Business Enterprise (CBE) Declaration Form
□ Annual Personal Net Income Attestation Form (if applying as a Medical Cannabis CBE)
□ Certification as a Medical Cannabis CBE from the Department of Small & Local Business Development (DSLBD)
(if applying as a Medical Cannabis CBE)
□ Non-refundable application fee
