Fire Officer Certification Application Form Instructions

Certification will not be issued unless documentation is received and validated.
Section
1. Provide your DFSID number, name, home address, county where you reside, contact telephone numbers and email.
2. Fire Officer Certification Requirements:
Level 1: a. Satisfactorily complete the Fire Officer I course of instruction adopted by the Division of Fire Safety in
accordance with N.J.A.C. 5:73-8, and successfully pass the State Fire Officer I exams that evaluate the Job
Performance Requirements (JPRs) and components of requisite knowledge and skills contained in the
adopted NFPA 1021 standard;
b. Certified as Firefighter I and Firefighter II issued by the Division of Fire Safety in accordance with N.J.A.C.
5:73-4;
c. Certified as Hazardous Materials-Awareness and Hazardous Materials-Operations issued by the Division of
Fire Safety in accordance with N.J.A.C. 5:73-6;
d. Certified as Incident Management Level 1 issued by the Division of Fire Safety in accordance with N.J.A.C.
5:73-3;
e. Satisfactorily complete the Fire Instructor I course of instruction adopted by the Division of Fire Safety in
accordance with N.J.A.C. 5:73-5, and successfully pass the State Fire Instructor I exams that evaluate the
Job Performance Requirements (JPRs) and components of requisite knowledge and skills contained in the
adopted NFPA 1041 standard; (Note: Fire Instructor I certification is not required for this credentialing
requirement, only the successful completion of the Fire Instructor I course and State exams.)
f. Satisfactorily complete a classroom instructor-led Incident Safety Officer course of instruction adopted by the
Division of Fire Safety in accordance with N.J.A.C. 5:73-8;
g. Satisfactorily complete at least 12 hours of training on Principles of Building Construction Series of
instructional courses which are similar in content with the National Fire Academy courses; and,
h. Satisfactorily complete at least 24 hours of training on Fire Tactics Series of instructional courses which are
similar in content with the National Fire Academy courses.
Level 2: a. Satisfactorily complete the Fire Officer II course of instruction adopted by the Division of Fire Safety in
accordance with N.J.A.C. 5:73-8, and successfully pass the State Fire Officer II exams that evaluate the Job
Performance Requirements (JPRs) and components of requisite knowledge and skills contained in the
adopted NFPA 1021 standard;
b. Certified as Fire Officer I issued by the Division of Fire Safety in accordance with N.J.A.C. 5:73-8; and,
c. Certified as Incident Management Level 2 issued by the Division of Fire Safety in accordance with N.J.A.C.
5:73-3.
Level 3: a. Satisfactorily complete the Fire Officer III course of instruction adopted by the Division of Fire Safety in
accordance with N.J.A.C. 5:73-8, and successfully pass the State Fire Officer III exams that evaluate the Job
Performance Requirements (JPRs) and components of requisite knowledge and skills contained in the
adopted NFPA 1021 standard; and,
b. Certified as Fire Officer II issued by the Division of Fire Safety in accordance with N.J.A.C. 5:73-8.
Level 4: a. Satisfactorily complete the Fire Officer IV course of instruction adopted by the Division of Fire Safety in
accordance with N.J.A.C. 5:73-8, and successfully pass the State Fire Officer IV exams that evaluate the Job
Performance Requirements (JPRs) and components of requisite knowledge and skills contained in the
adopted NFPA 1021 standard; and,
b. Certified as Fire Officer III issued by the Division of Fire Safety in accordance with N.J.A.C. 5:73-8.
3. The application form must be signed and dated.
